This form declares your current health status, travel history, contact details, and address in India. By filling this form online, you can avoid the hassle of filling a physical form at the airport and reduce the risk of exposure to COVID-19.


Air Suvidha helps you avoid mandatory institutional quarantine




As per the latest government guidelines, all international passengers arriving in India are required to undergo 14 days of quarantine, out of which 7 days are in an institutional facility and 7 days are at home. However, if you meet certain criteria, you can apply for exemption from institutional quarantine through Air Suvidha. These criteria include:


  • Pregnant women



  • Suffered a death in the family



  • Suffering from serious illness (Description to be provided)



  • Parents accompanied by children below 10 years



  • Covid-19 negative certificate (attach RT PCR test only)



If your exemption request is approved by the respective state authorities, you can skip the institutional quarantine and go directly to your home or hotel for 14 days of self-monitoring.

How to fill and submit Air Suvidha form online?




Filling and submitting Air Suvidha form online is easy and takes only a few minutes. You can do it by following these steps:


Step 1: Visit the Air Suvidha portal

Step 2: Choose the type of form you need




On the Air Suvidha portal, you will see two options: Self Declaration Form and Exemption Form. You need to fill and submit both forms if you are requesting for exemption from institutional quarantine. Otherwise, you only need to fill and submit the Self Declaration Form.


The Self Declaration Form is mandatory for all international passengers arriving in India. It asks for your personal details, travel history, contact details, address in India, and health status. You also need to agree to abide by the government guidelines and regulations regarding COVID-19.


The Exemption Form is optional and only for those who meet the criteria for exemption from institutional quarantine. It asks for your personal details, travel details, exemption category, and supporting documents. You also need to provide a valid reason for your exemption request.


Step 3: Enter your personal and travel details




After choosing the type of form you need, you will be directed to a page where you need to enter your personal and travel details. These include:


  • Your name, gender, age, nationality, passport number, visa type, and visa number



  • Your flight number, origin country, destination city, arrival date, and arrival time



  • Your email address and phone number



  • Your address in India and the name and phone number of your local contact person



Make sure that you enter all the details correctly and accurately. Any mismatch or discrepancy may lead to rejection of your form or exemption request.


Step 4: Upload the required documents




If you are filling the Exemption Form, you need to upload the required documents to support your exemption request. These documents vary depending on your exemption category. For example:


  • If you are pregnant, you need to upload a medical certificate or a pregnancy test report



  • If you have suffered a death in the family, you need to upload a death certificate or a letter from the hospital



  • If you are suffering from a serious illness, you need to upload a medical prescription or a doctor's letter



  • If you are accompanied by children below 10 years, you need to upload their birth certificates or passports



  • If you have a COVID-19 negative certificate, you need to upload an RT PCR test report done within 72 hours before boarding



The documents should be in PDF format and should not exceed 1 MB in size. You can upload up to four documents per exemption category.
